What is My EAH?
My EAH is a free, secure and private online patient portal where you can access your medical information whenever and wherever you need it.

How to Sign Up for My EAH:
At East Alabama Health, we value your privacy and take great measures to protect your information. Please ensure that you include an accurate and up-to-date email address, preferably before you leave your hospital visit or before you register online or in person to ensure that you will receive an email invitation to My EAH.
After your hospital visit:
- Patients who are age 19 years or older who do not currently have access to My EAH Record can go to https://myeamcrecord.iqhealth.com/self-enroll/ to self-enroll.
- When registering, please use the same email address as provided to EAH during your visit. It will be required for you to access your My EAH account.
- After you self-enroll, you will receive an email from noreply@iqhealth.com. Be sure to check your spam folder if you don’t see the email in your inbox.
- Click the blue link in the email. You will then enter your birthday and the last four digits of your social security number to gain access to your My EAH account.
Without a hospital visit:
- Visit the Medical Records Department at East Alabama Medical Center (EAMC) with your photo ID and a completed patient portal authorization form. A valid email address is required for accessing your patient portal.
- If you can’t make it to the Medical Records Department, you can mail or fax the form with a copy of your picture ID.
