Subscribers and spouses who are covered through our BCBS East Alabama Health and Healthlinks insurance plan are REQUIRED to complete a preventive office visit and specific labs annually, along with age-specific requirements for colon and breast cancer. Gaps in Care for 2023 runs from August 1, 2022 through July 31, 2023. This applies to the member and their spouse (dependents are NOT required to complete Gaps in Care).

Please refer to this attachment for required labs and age-specific gap requirements. The attachment CANNOT be used to get your lab work done. You must have a physician order. The attachment is simply for you to ensure that the physician order matches the info. on the required labs.

Print the attachment and take it with you to your appointment to be sure your provider orders all the necessary tests. (NOTE: If your doctor requires you to get lab work PRIOR to your appointment, take the attachment with you when you visit the lab to ensure they order all the necessary tests.) Failure of both the employee and spouse to complete Gaps in Care by July 31 will result in an increased insurance premium for the 2024 calendar year.
