Diabetes Self-Management Training
At the Diabetes and Nutrition Center (DANC), we provide personalized care for each individual living with diabetes, including both adults and pediatrics. Our team of diabetes care and education specialists, along with your doctor, will help you integrate eating and physical activity into your lifestyle. We also assist in understanding how your medications work, teach you how to monitor your blood sugar, use diabetes technology, and provide emotional support to help you manage challenges. Throughout your journey, we'll send status updates to your referring physician to ensure continuous progress, monitoring and care.

Helping You Manage Your ABC’s
To manage diabetes, you will work with your health care team to make a plan that helps you reach your goals. Together, you'll keep track of the ABCs of diabetes:
A is for A1C: Your A1C test shows your average blood sugar level over the past 2-3 months. Think of it as a "blood test with a memory."
B is for blood pressure: Your blood pressure measures how hard your heart is working to pump blood through your blood vessels. High blood pressure means your heart has to work harder.
C is for cholesterol: Your cholesterol levels show the amount of fat in your blood. HDL cholesterol is the "good" type that protects your heart, while LDL cholesterol can build up in your blood vessels and cause heart problems. Triglycerides are another type of fat that can increase your risk for heart attack or stroke.
