How to refer a patient for pulmonology care
We make the referral process straightforward so providers can quickly connect patients with East Alabama Pulmonology.
Options for sending a patient referral:
- Fax: (334) 528-6671
- Electronic Health Record system
Please include the following with your faxed referral:
- CT scan reports and images
- X-ray scan reports and images
- Brief patient medical history
Once a referral is received by fax or through East Alabama Health’s EHR platform, our team reviews it to confirm it is for a pulmonology-related condition, then contacts the patient to schedule an appointment with the appropriate provider.
